Choosing a Primary Care Physician
Choosing a primary care physician is an important decision. Many of our physicians are happy to make a preliminary appointment where you can ask questions and learn more about him or her, to help you make a decision about the best primary care physician for you and your family.
Once you select your physician, the following information can help you transition to your new PCP.
- Alert your insurance company that you have chosen a new PCP. Be sure to have your new PCP’s phone number and address available when you call your insurance company.
- Call your previous practice and ask that your medical records be transferred to your new physician’s practice.
- Make a list of your current medication and any questions you want to discuss with your new physician.
