Parents, physicians, schools and other professionals may request an appointment on behalf of a child, adolescent or young adult.
Intake Form for younger patients
The intake form helps us gather valuable information necessary to determine the type of services needed. Any additional information which you believe might be relevant, such as other recent evaluations (e.g., school or CORE evaluation, prior psychological or neuropsychological testing reports), special education plans (Individual Education Plans, or IEPs), or recent therapy reports, should also be sent.
- We will contact you if we have any initial questions.
- As soon as the intake form is received, you will be placed on our wait list. Please inform us if testing is scheduled or being requested elsewhere in the near future (e.g., through school).

Insurance
- Your insurance company may cover the cost of this initial meeting. Please check with them prior to the interview.
- Most insurance companies require prior authorization for psychological or neuropsychological testing.
- The hospital accepts most Massachusetts insurance plans, and alternative options for covering these evaluations are discussed during the interview.
- NOTE: most insurance companies do not pay for testing that is being done solely for educational purposes. Once authorization from the insurance company is obtained, the actual evaluation will be scheduled.
