Salem, Mass. – North Shore Medical Center (NSMC) has launched a new outpatient diabetes management program, based in the Gloucester office of Cape Ann Medical Center. Approved by the American Diabetes Association, the program teaches patients with diabetes how to better manage their diabetes through lifestyle choices, medication and continuous monitoring.
"We’re pleased to be able to offer our patients closer access to NSMC’s highly effective outpatient diabetes education program," said Jay Isaac, M.D., physician and president of Cape Ann Medical Center. "Patients who are better educated about how to effectively manage their diabetes have fewer complications, hospitalizations and a better quality of life. We’re recommending all our diabetic patients enroll in the program, which is the embodiment of our dedication to providing access to high-quality outpatient care locally."
Patients participate in a five-session educational program that covers topics including how to monitor blood glucose levels, how to properly administer medications, nutrition, complications of diabetes, exercise and stress management, meal planning and tips for how to eat when away from home. This program is reimbursed by many health insurance policies (referrals from your primary care physician may be needed). New sessions start monthly and are held on Tuesday evenings from 4 to 5:50 p.m. Patients can call 781-477-3409 to register.
