All surgical patients will now be able to complete the medical history portion of their registration online, at their own convenience.

If you are scheduled to have surgery at the Mass General/North Shore Center for Outpatient Care, or at either NSMC Salem Hospital or NSMC Union Hospital, you must submit your medical history online at the secure One Medical Passport web site.

Pre-Surgery Assessment

If you are having surgery at the Mass General/North Shore Center for Outpatient Care, your pre-surgery assessment will take place via a phone call.

If you are having surgery at either NSMC Salem Hospital or NSMC Union Hospital, your pre-surgery assessment will depend on the nature of your surgery and your overall health.
