Application Process

Volunteers must submit an application to volunteer. The application below can be printed out, filled in and mailed to the address given. Before you submit your application, please read our volunteer requirements.
Volunteer application process at North Shore Medical Center

  • Potential candidates are required to fill out an application and interview with a member of the Volunteer staff.
  • All volunteers are required to receive a TB (tuberculin) test. They also must provide documentation of childhood immunizations (if available) or submit to blood work. Both TB test and blood work are provided by the hospital free of charge.
  • All volunteers must complete CORI (Criminal Offender Record Information) and confidentiality forms, receive a hospital photo ID and go through the hospital orientation process in order to become familiarized with hospital policies.
