Heart Failure Clinic

The Living Well with Heart Failure Clinic is a key component of the North Shore Medical Center (NSMC) Heart Center. Our team includes cardiologists, nurse practitioners, heart failure nurse coordinators, dietitians, case managers, and home care specialists. Team members work closely with patients and their primary care physicians throughout the process. Our outpatient clinic, offered by NSMC’s cardiovascular colleagues, provides comprehensive education to heart failure patients, enabling them to successfully manage their illness after they leave the hospital.

Heart Failure Clinic

The clinic’s team of caregivers works closely with patients, their families and physicians to provide support and help manage their heart failure condition. Lifestyle choices, medication and continuous monitoring are all components of the program that help Heart Center patients to:

  • Decrease their heart failure symptoms
  • Increase their exercise capacity
  • Improve their quality of life
  • Slow progression of the disease
  • Improve their long-term survival

Most insurance companies cover the cost of this clinic. Heart Center patients may join the Living Well with Heart Failure clinic or have a physician refer them.
