Medical Records

Patients who would like to obtain a copy of their medical records for care received at NSMC Salem Hospital or NSMC Union Hospital are asked to download and fill out one of the following forms: Release of Information form or Spanish version of Release of Information form. Completed forms can dropped off in person or mailed. Faxes are not accepted. Please be aware that a fee may be associated with the request and that requests are usually processed within five to ten business days.

Copies of Protected Health Information will not be given unless one of the following forms of identification is provided.

  • Valid U.S. Driver’s License; or
  • Valid U.S. State-issued non-driver identity card; or
  • Valid U.S. Passport
  • Expired identification cannot be accepted

If a patient is a minor or is not competent to give consent, the signature of a parent, guardian or other legal representative is required.

For deceased patients, additional documentation will be required for release of protected/privileged health information.
