Medical Records

Patients who would like to obtain a copy of their medical records for care received at NSMC Salem Hospital or NSMC Union Hospital are asked to download and fill out one of the following forms: Release of Information form or Autorizacion para reciber informacion medica protegida.
  • Completed forms can be dropped off in person or mailed.
  • Faxes are not accepted.
  • A fee may be associated with the request
  • Requests are usually processed within five to ten business days.
Forms of identification required for protected health information (choose one):

  • Valid U.S. Driver’s License; or
  • Valid U.S. State-issued non-driver identity card; or
  • Valid U.S. Passport
  • Expired identification cannot be accepted

If a patient is a minor or is not competent to give consent, the signature of a parent, guardian or other legal representative is required.

For deceased patients, additional documentation will be required for release of protected/privileged health information.
