Your medical history, including test results from pathology, radiology, labs, as well as operative reports of treatments and medical services, constitute your medical records.
Patients can request copies of their medical records, and may also request that amendments be made, by submitting forms to the Partners HealthCare Medical Records Department. Note that there are separate forms for NSMC Salem Hospital vs. NSMC Union Hospital. Forms are in English and in Spanish.
Urgent in-Person Requests
If you have an urgent need of copies of your medical record or have questions regarding your request, please contact 617.726.2361.
You may also visit the medical records office at NSMC Salem Hospital. The office is located on the fifth floor of the Axelrod Building. If your records are at NSMC Union Hospital, the office is located in the basement of the Hartman Building.
Patients who are minors or who cannot give consent:
If the patient is a minor or unable to give consent, the signature of a parent, guardian or other legal representative is required. Additional authorization may also be required for the release of specifically protected or privileged information. Certain information can take up to 30 days for processing.

- North Shore Medical Center does not provide birth or death certificates.
- To request medical records of a deceased patient, the request must be accompanied by authorization from the executor of the estate.
